To complete Student Registration at Albion Central School - forms
are available in the Registration Office of the ACS District Office.
You will need to provide the Registrar's Office with the following
information.
Original Documentation
• Birth Certificate
or
• Hospital Certificate
Record of Immunizations:
• Dates for 3 DPT or DT
** NEW 9/2007 - 6th grade students DTaP Booster 11 yrs old
• Dates for 3 Oral Polio
• Dates for 2 MMR (Measles-Mumps-Rubella)
• Dates for 3 HepB (2 for Adult dose)
• 1 dose Varicella (chicken pox) or physician verification
of disease.
Children born on or after January 1, 1998 AND ALSO students
entering 6th , 7th & 8th grades (began with 6th grade entrants
in the Fall of 2005)
Copy of previous school's report card or academic record to date
of leaving. This information is not mandatory, but helpful. (Student
must be withdrawn from previous school)
Does the child live with natural parent?
Custody documents must be provided if the
student does not live with either natural parent. Only Family
Court documents accepted.
